CUSTOM CARDS

Our primary focus is Adult Orthopaedics. We specialize in making personalized Hip or Knee Replacement Id Cards for total Hip & Knee Arthroplasty; partial and Bilateral Implants. We also care for Shoulder, Foot, Spine, Defibrillator and Pacemaker implants with standard implant card (see examples below). Additionally we Custom make Medical Alert ID Cards using the patients actual x-ray. We can make standard and custom cards to meet most implant surgeries:

Custom Cards are made for your specific implant procedure.

If our standard card file does not meet your needs we can use your actual x-ray. You will need to provide specific surgery data on the order form and send us your x-ray (available at your doctor) or a digital copy (via internet). X-rays are not returned.

Cost is $40.00 for primary Custom card and $18.00 for each additional copy.

Avoid close or sustained contact with metal detectors and hand wands; there is a  risk that electromagnetic interference can cause implanted device to malfunction. Pacemaker could reset; Defibrillator could deliver a painful shock. Avoid Pressing or pressure to surgical areas.

Handicap Cards. We can make you a wallet sized "Handicap" card similar to our id cards. You can order the RED temporary handicap or BLUE permanent handicap card.
It is best to send us a color fax of your handicap card issued by your DMV. Or, we can make you a generic handicap card to describe your condition.
