CARD
is
a great way to visually show and your joint replacement surgery.
The "Implant Alert id card" is a
laminated wallet sized
card about 2" wide and 3-1/4th " long.
Your personalized data are transferred to our publishing
program which sizes it and prints it to a standard wallet
card
size. The card with your data, plus the photo of a
standard hip (or knee) implant x-ray and a hip (or Knee)
prosthesis
are laminated together, front and back.
The result is a personalized and long lasting ID card
detailing your total hip or total knee replacement
surgery.

Pre-paid Mailed orders are $17.00 for the first card, $8.00 each
extra card on same order when
Pre-Payment is made by check or money order.
Internet/ Paypal orders are $18.00 for the first card which includes
a $1.00 paypal handling fee added at checkout.
